Financial assistance for family building
We believe that everyone should have equal access to high quality fertility care, but also recognize that for many, affording treatment can be difficult.
Which is why Pollin is proud to partner with the TheModern Miracle Foundation, a registered Canadian charity that provides grants to individuals and families struggling to afford fertility treatments, helping ease their financial stress and making fertility treatments more accessible for them.
Together, we have created the Pollin x Modern Miracle IVF Grant Fund, which awards an (up to)$8000 IVF Grant to Pollin patients who qualify. For every grant awarded, Pollin will also contribute (up to) $2000 in additional funds, bringing the total grant amount to $10,000.
Eligible treatments include:
Offset the cost of 1 x IVF cycle
*Cover the cost of 1 x Frozen Embryo Transfer of previously frozen embryo
*Cover the cost of 1 x Egg Thaw/Fertilization cycle of previously frozen eggs + transfer (FET or Fresh)
*Applicant is only eligible for 1 grant for 1 of the above treatment options
*All grants exclude the cost of medications. For a full list of exclusions, please see the FAQ section below.
*Grant is only eligible to cover 1 cycle/treatment

The (up to) $10,000 grant is a gift, repayment is not necessary
There is a $50 application fee (treated as a donation) to ensure the applicant is committed
The funding will be provided by Modern Miracle to Pollin, not to the recipient directly
The grant must be used within 6 months of application approval date and only apply to IVF treatment
The grant will not apply to any OFP funded cycles
We utilize a set of criteria to ensure a fair and objective selection process.
Applicant and co-applicant (If applicable) must be a Canadian citizen or permanent resident, be a current resident of Ontario & have a valid OHIP card

Both the applicant and co-applicant (if applicable) must be a Canadian citizen or permanent resident and a current resident in the province of Ontario
Both the applicant and co-applicant (if applicable) must have valid Ontario Health Care Plan cards (OHIP)
You must already be a Pollin patient and have completed a consultation and fertility assessment
Combined (if applicable) annual gross household income of <175K

All applicants are required to complete and upload 3 documents along with an online application and submit a $50 application fee which goes directly to the fund. Instructions along with the documents and online application link are all found on the Pollin x Modern Miracle IVF Grant Fund page.
The grant can be used to:

- Includes cycles using donor egg, sperm or embryo
*Cover the cost of 1 x Frozen Embryo Transfer of previously frozen embryo
*Cover the cost of 1 x Egg Thaw/Fertilization cycle of previously frozen eggs + transfer (FET or Fresh)
*Applicant is only eligible for 1 grant for 1 of the above treatment options

Expenses not covered by the grant include:
Medications
Donor, surrogate agency and legal fees
Transfer fees of previously frozen eggs, donor egg, sperm or embryos to Pollin
Embryo storage fees
Advanced diagnostic/genetic testing fees
Yes, grants must be applied to your invoice and used within 6 months of your application approval date.
Each application is reviewed by members of the Modern Miracle team, including Medical Advisors. We verify the information provided in the application form as well as the Medical Evaluation signed by your physician.
Personal information in the Medical Evaluation form is redacted to eliminate bias and doctors will never review and opine on their own patients.
There are many considerations that go into the applicant selection process. For example, we review financial need as well as the prognosis provided by your doctor. All of these considerations help us with the difficult decision of who is selected for a grant.
While we aim to review applications as quickly as possible, it may take up to 8 weeks to complete the review.
Once the application review is completed, you will be contacted via email as to the outcome of your application.
You can re-apply but we suggest only re-applying if there has been a material change in your application. For example, if your application is rejected due to medical prognosis, we recommend applying if there has been a material change to your treatment plan (eg. from using own egg/sperm to using donor).

These grants are one-time grants. As such, you cannot re-apply after successfully receiving a grant from the Pollin x Modern Miracle IVF Grant fund.
